29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On or about May 23, 2019, at the above addressed jobsite, employees were performing roofing operations approximately 13.1 feet above ground and were not using fall protection.   The employees were exposed to fall hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about May 23, 2019 at the above addressed jobsite, a portable straight ladder was used to access the roof that was 13.1 feet from ground and the ladder did not extend at least three (3') feet above the landing, thereby exposing employees to hazards associated with falls.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
