On or about May 22, 2019, an employee of Harrington Electrical Contractors was using a job made aluminum basket that was not secured to the lift while lifting personnel.  The employee could not shut off power to the lift if needed, exposing employees to struck-by hazards.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    a. On or about May 22, 2019, a competent person did not inspect a job-made aluminum basket being used by ab employee of Harrington Electrical Contractors.  The basket on the left side had the midrail missing , the frame had cracks and the basket was not tied to the mast, exposing employees to fall hazards.

On or about May 22, 2019, a competent person did not remove from service a job-made aluminum basket being used by subcontracted employees of Harrington Electrical Contractors.  The basket on the left side had the midrail missing , the frame had cracks and the basket was not secured to the lift, exposing employees to struck-by hazards.
