29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about May 14, 2019 at the southeast side of the ne construction Touchton Office Building, two employees did not wear a body belt and lanyard attached to the boom while operating an extendable reach straight boom areal lift, exposing employees to fall from elevation to lower surface hazards.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On or about May 14, 2019 at the southeast side of the new construction Touchton Office Building, two employees were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems while working on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels.
