29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.    A work related incident which resulted in an employee hospitalization was not reported to OSHA within 24 hours. The incident occurred on or about May 08, 2019 at 1:30 am and was reported to OSHA on May 14, 2019 at 8:30am.

29 CFR 1910.303(g)(1):  Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.    Managers and employees accessing the store office were exposed to electric shock and fire hazards. The electrical panels in the office and back storage room did not have at least 3 feet of clearance as required in the wiring design and protections standard. The electrical panels are completely blocked by boxes of merchandise.
