29 CFR 1910.146(c)(5)(i)(F): Entry into the permit space under the terms of 29 CFR 1910.146(c)(5)(i) was not performed in accordance with the requirements of 29 CFR 1910.146(c)(5)(ii) of this section:    a)  Wright Choice, Inc., Canadian Slang, Maple Pad, Ault, CO, 80610:  On or before May 22, 2019, entry into the permit space under the terms of 29 CFR 1910.146(c)(5)(i) was not performed in accordance with the requirements of 29 CFR 1910.146(c)(5)(ii) of this section.  The employer did not adequately test the air in the tanks prior to entry in that the employer did not test the air at the bottom of the tank.  Continuous forced air ventilation was not used.  The employer did not verify that it was safe for entry through a written certification.  These conditions exposed employees to amputation and asphyxiation hazards.

29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146:    a)  Wright Choice, Inc., Canadian Slang, Maple Pad, Ault, CO, 80610:  On or before May 22, 2019, the employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146.  The employer allowed employees to enter into permit required confined spaces without adequate training on testing the air, on continuous forced air ventilation, on lockout tag out procedures, and on the documentation needed to support the use of alternative procedures for safe confined space entry under 1910.146(c)(5).  This condition exposed employees to amputation and asphyxiation hazards.

29 CFR 1910.151(b): In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which is used for the treatment of all injured employees, a person or persons were not adequately trained to render first aid.  Adequate first aid supplies were readily available:    a)  Wright Choice, Inc., Canadian Slang, Maple Pad, Ault, CO, 80610:  On or before May 22, 2019, the employer did not ensure that employees were trained in first aid and medical treatment in the absence of an infirmary, clinic, or hospital in near proximity to the workplace.  An employee was injured when cleaning mud tanks that included hydrated lime.  A co-worker transported the employee to the shop, and then another employee transported him to a hospital approximately 90 minutes after leaving the Maple Pad.  These employees were not adequately trained on procedures for handling chemical burns and chemical inhalation.  This condition exposed employees to chemical hazards.

An employee cleaning the mud tanks for Rig #147 did not wear adequate protective equipment for chemical burns and respiratory hazards.  The employee wore a Tyvek suit and fire retardant clothing, but experienced first degree chemicals burns and vomiting that required hospitalization.  This condition exposed this employees to chemical burn and respiratory hazards.
