29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  On or about May 29, 2019, the employer failed to ensure the employee that was engaged in tearing off the shingles was protected from a fall to the lower level. The employee was exposed to a fall hazard of approximately 12 feet to the cement sidewalk below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about May 29, 2019, the employer failed to ensure the employee was trained to recognize the fall hazards and procedures to be followed in order to minimize the hazards.  The employee was tearing off shingles on the roof. The employee was exposed to a fall hazard of approximately 12 feet to the cement sidewalk below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  On or about May 29, 2019,  the employee was gaining access to the roof from a step ladder. The combination ladder made by Gorilla, model number MPX26, was made into a step ladder. The employee was stepping sideways off the step ladder to gain access to the roof. The employee was exposed to a fall hazard of approximately 8 feet to the cement sidewalk below.

The employee was ascending and descending the roof from a combination ladder made into a step ladder. The combination ladder was a Gorilla, model MPX26. The employee was exposed to a fall hazard of approximately 8 feet to the cement sidewalk below.
