29 CFR 1910.147(c)(2)(ii): If an energy isolating device is capable of being locked out, the employer's energy control program under paragraph (c)(1) of this section shall utilize lockout, unless the employer can demonstrate that the utilization of a tagout system will provide full employee protection as set forth in paragraph (c)(3) of this section:  On or about May 24, 2019, the employer failed to ensure that locks to de-energize the company's rotary furnace located in the Precision Forge Department were used by employees when completing service and maintenance activities at the facility. On May 16, 2019, an employee in the process of servicing a rotary furnace sustained an amputation injury, when his finger was caught in the ingoing nip point of the belt drive motor of the rotary furnace table.   Sifco Forge Group dba Sifco Industries Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1910.147(c)(2)(ii), which was contained in OSHA inspection # 1010588, citation 1, item number 2A and was affirmed as a final order on November 18, 2015, with respect to a workplace located at 970 E. 64th Street in Cleveland, OH, 44103.
