Employees used Ecolab Grease Strip Plus (containing 5-10% sodium hydroxide) to clean ovens.  Eye protection was not used during this activity.  The employer did not ensure that an emergency eyewash was provided within the work area.  This condition exposed employees to a chemical burn hazard.

29 CFR 1910.1030(c)(1)(i):  The employer had not established a written Exposure Control Plan designed to eliminate or minimize employee exposure to bloodborne pathogens:    (a)  SRC of Colorado LLC, dba HighPointe Assisted Living & Memory Care, 6383 E Girard Place, Denver, CO: On and preceding 5/29/19, the employer had not established a written Exposure Control Plan designed to eliminate or minimize employee exposure to bloodborne pathogens.  Employees assisted a resident with administration of insulin using a Levemir FlexTouch insulin injector with BD Ultra-Fine Mini Pen Needles.  Employees disposed contaminated sharps.  Employees performed tasks, including housekeeping and responding to spills of blood.  The employer had not established a written Exposure Control Plan that addressed all the required elements.  This condition exposed employees to a bloodborne pathogens hazard.    (b)  SRC of Colorado LLC, dba 6383 E Girard Place, Denver, CO: On and preceding 5/29/19, the employer did not ensure that the bloodborne pathogens training program contained an explanation of the use or limitations of methods that would prevent or reduce exposure including appropriate engineering controls, work practices or personal protective equipment.  Employees assisted a resident with administration of insulin using a Levemir FlexTouch insulin injector with BD Ultra-Fine Mini Pen Needles.  Following injection of insulin, employees removed the contaminated needle prior to disposal.  The employer's bloodborne pathogen training did not include training with regards to the medical device.  This condition exposed employees to a bloodborne pathogens hazard.

Employees were exposed to chemical splash, flying object, and chemical burn hazards during housekeeping, kitchen, and maintenance tasks.  Hazards included, but were not limited, to the following:    1.    Exposure to liquid chemical splashes to the eyes or skin when performing housekeeping duties using Zep Acidic Toilet Bowl Cleaner, Clorox Bleach Toilet Bowl Cleaner, Ecolab Peroxide Multi-Surface Cleaner and Disinfectant, Ecolab Heavy Duty Alkaline Bathroom Cleaner and Disinfectant, or Easy Off Glass Cleaner;  2.    Exposure to chemical burn to the eyes or skin when using Ecolab Greasestrip Plus to clean kitchen ranges; and  3.    Exposure to flying objects when performing maintenance on facility equipment.
