29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury:    a) 8 Broadway Avenue, West Chester, PA: Employees who were working in an excavation without any protective system in place and without a safe means of egress were exposed to hazards such as cave-ins and falls in that, the employer did not provide training on recognizing and correcting unsafe work conditions.    Condition was observed on or about 29 MAY 2019.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:  a) 8 Broadway Avenue, West Chester, PA: Employees working in a 13 foot deep excavation were exposed to struck-by/caught between hazards in that, the spoil pile closer than 2 feet from the excavation edge.  Condition was observed on or about 29 MAY 2019.
