29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  a) On or about 05/30/2019, at the storage area for compressed gas cylinders in the fabrication shop:      1) Compressed gases of different types were stored together at the same location.      2) There were no signs posted indicating the identity of the substance and the hazard associated with flammable compressed gas cylinders being stored.      3) Compressed gas cylinders in storage were missing protective caps.  b) On or about 05/30/2019, at the fabrication shop, an argon compressed gas cylinders was not properly secured to prevent for being knocked over.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) On or about May 30, 2019, at the fabrication shop, employees performing cleaning, clearing jams or setting up Accurpress 725012 and Accurshear 625012 were exposed to caught in, caught in between and struck by hazards, in that the employer did not have a hazardous energy control program.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  a)  On or about May 30, 2019, at fabrication, employees were using a powered hand grinder, polisher and sander with a damaged cord that had visible internal wiring exposed.  b)  On or about May 30, 2019, at fabrication, employees were using an extension cord to power different hand tool equipment where the outer cover was pulled from the strain relief, exposing wires near the plug connector.  c)  On or about May 30, 2019, at fabrication, employees were using a damaged extension cord to power portable equipment where the outer cover was split open exposing wiring near the plug.
