The employer did not instruct employees working at this site that hazardous levels of carbon monoxide could likely be produced while operating combustion engine equipment inside an enclosed insulated warehouse, and the method and means to avoid overexposure.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

The employer did not properly train and provide to employees working at this site methods and observations used to detect the presence or release of carbon monoxide in the work area (such as monitoring), the health hazards, and the measures employees could take to protect themselves from carbon monoxide.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.       ABATEMENT NOTE:   Employees shall be informed of the following:  1. The requirements of this section;  2. Any operations where hazardous chemicals and/or gases are present;  3. Methods and/or equipment the employer will provide to employees to detect and prevent overexposures to carbon monoxide.  4. The location and availability of the written Hazard Communication Program, list(s) of hazardous chemicals and Safety Data Sheets.
