Located at 11533 Sumner Way, Plain City, Ohio: On or about May 31, 2019, the employer did not initiate and maintain a safety and health program to ensure hazard detection and correction, while employees performed sheathing work onsite. Two employees were unprotected from the roof top of the third level of a new build residential home. The fall distance was approximately 21 feet from the roof's eave to the ground level.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. a. Located at 11533 Sumner Way, Plain City, Ohio: On or about May 31, 2019, the employer failed to ensure the use of eye protection, while an employee installed oriented strand board (OSB) roof sheathing using a pneumatic nail gun, to the roof top a new build residential home. The employee was exposed to a struck by hazard as a result.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502. a. Located at 11533 Sumner Way, Plain City, Ohio: On or about May 31, 2019, the employer failed to ensure that two employees who worked from the roof top of a new build residential home were protected from falls, while they installed OSB sheathing. The employees were exposed to a fall hazard of approximately 21 feet, without any means of fall protection.
