29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards were not provided with, and did not use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed. Note: Personal protective equipment requirements are contained in subpart I of this part. On or about May 21, 2019. Employees were exposed to arc flash or electric shock by not wearing appropriate PPE when working with 480 volts when installing a fuse on a power disconnect for a crane.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. Employer failed to report to OSHA a work related hospitalization of an employee prior to the elapse 24 hour reporting period of the incident.
