29 CFR 1910.29(b)(1): The top edge height of top rails, or equivalent guardrail system members were not 42 inches, plus or minus 3 inches, above the walking-working surface:    1) On or about June 5, 2019, and at prior times, employees were exposed to fall hazards in excess of 12 feet where a chain used as a top rail of the guardrail system on top of the CO2 extraction machine was deflected approximately 35" above the walking working surface.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    1) On or about May 29, 2019, and at prior times, employees were exposed to the hazard of unexpected start up or release of stored energy when servicing the CO2 extractor (kracken machine).  The CO2 extractor had multiple sources of energy and the employer had not developed and documented written procedures for shutting down, isolating, or blocking the machine prior to maintenance and service activity.
