(b)  On or about May 28, 2019, the employer did not train employees to recognize and mitigate the hazards of lightning.  The employees were working on a frame scaffolding during a thunderstorm with lightning.  The workers were not trained to recognize the hazards from lightning such as death, electrocution and burns, and to take actions to mitigate the hazards by staying off of the scaffolding until the storm left the area.
