29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates, mud sills or other adequate firm foundation:  (a)  On or about May 28, 2019, and at times prior, employees were exposed to fall hazards while working on a scaffolding that did not have mud sills under every base plates, which were placed on the dirt around a house that was under construction.  The base plates were sinking into the wet dirt in several locations. 29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  (a)  On or about May 28, 2019, and at times prior, employees were exposed to fall hazards while working on a scaffolding that was not inspected by a competent person before the shift began.  The scaffold had many hazardous conditions that a competent person needed to have identified and corrected before the scaffold was fit for use. 29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a)  On or about May 28, 2019, and at times prior, the employer did not train employees to recognize and mitigate fall hazards while working from frame scaffolding.  The working platform of the scaffolding did not have guardrails and the employees were exposed to falls of 14 feet.  (b)  On or about May 28, 2019, the employer did not train employees to recognize and mitigate the hazards of lightning.  The employees were working on a frame scaffolding during a thunderstorm with lightning.  The workers were not trained to recognize the hazards from lightning such as death, electrocution and burns, and to take actions to mitigate the hazards by staying off of the scaffolding until the storm left the area. 29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  (a)  On or about May 28, 2019, workers were exposed to fall hazards while using a ladder to access the second level of a scaffold, that did not extend at least three rungs above the scaffold planking, was not secured, and did not have grasping devices to assist employees in maintaining balance.
