Location: Warehouse On or about May 29, 2019, the following workplace injuries and illnesses were not recorded on the OSHA Form 300 or equivalent for calendar year 2019. a) On or about May 18, 2019, an employee was struck by two crates of window panes that resulted in a torn rotator cuff and the employee remains out of work. b) On or about May 18, 2019, an employee was struck by two crates of window panes that resulted in traumatic iritis to the left eye, a concussion, bruising and swelling to the head and shoulder, and post-traumatic syndrome symptoms.
