29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On June 3, 2019; at least four employees were re-roofing a two-story home at 1002 Windsor Lane, Forney, TX approximately eighteen (18) feet or more above the ground below and were not protected from a fall hazard by using either guardrail systems, safety net systems, or personal fall arrest systems. (b) On June 3, 2019; at least two employees were re-roofing the single story garage of a home at 1002 Windsor Lane, Forney, TX approximately nine feet or more above the ground below and were not protected from a fall hazard by using either guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed; the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: (a) On June 3, 2019, at least four employees were using an aluminum extension ladder at a worksite located at 1002 Windsor Lane, Forney, TX, which had its side rails approximately six inches above the upper landing thereby exposing the employees using it to a fall hazard of approximately eighteen (18) feet to the ground below. (b) On June 3, 2019, at least two employees were using an aluminum extension ladder at a worksite located at 1002 Windsor Lane, Forney, TX, which had its side rails approximately one foot above the upper landing thereby exposing the employees using it to a fall hazard of approximately nine feet to the ground below.
