29 CFR 1910.23(c)(11): (11) Portable ladders used to gain access to an upper landing surface have side rails that extend at least 3 feet (0.9 m) above the upper landing surface (see Figure D-1); On June 4, 2019, at 601 S. Keowee St. Dayton, a two story brick building, four employees were exposed to a 25 and 30 ft. fall hazard while ascending and descending a metal extension ladder with its side rails extending only 6 inches above the top of the parapet wall of the roof.

(i) When work is performed less than 6 feet (1.6 m) from the roof edge, the employer must ensure each employee is protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system. On June 4, 2019 at 601 S. Keowee St. Dayton, a two story brick building, four employees were exposed to a 30 ft. fall hazard while repairing the rubber covering on the low slope roof within 6 feet of the edge of a parapet roof measuring 33 inches in height without utilizing any fall protection.

29 CFR 1910.30(a)(1): (a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart. Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. The employer had not trained employees before being exposed to a fall hazard on June 4, 2019, at 601 S. Keowee St., Dayton, a two-story brick building, on how to install a fall protection system (guardrail system) on a low slope roof with a parapet wall measuring 33 inches in height while repairing the rubber covering on the roof.
