29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:    (a) Peerless Hardware - The employer did not perform initial monitoring when the noise level for the cut off saw area, tumbler area and the grinding area indicated that employee's noise exposure equal or exceed the average to 85 decibels, on or about June 4, 2019.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1904.29(b)(3): Each recordable injury or illness on the OSHA 300 Log and 301 Incident Report was not recorded within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.    (a) Peerless Hardware Manufacturing Company - The employer did not record an injury that occurred on June 26, 2019 onto the OSHA 300 log within seven (7) calendar days, on or about August 23, 2019.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
