29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Main Roof, Residential Building, 138 West Spring Street, Frackville, PA - An employee on a 6:12 pitch main roof engaged in roof replacement activities of the residence was not protected from falling by guardrail system, safety net system or personal fall arrest system, exposing the employee to a 10 foot fall hazard, as observed on or about June 5, 2019.    b) Attached Room Roof, Residential Building, 138 West Spring Street, Frackville, PA - An employee on a 3:12 pitch roof of an attached room to the main residence was engaged in roof replacement activities was not protected from falling by guardrail system, safety net system, or personal fall arrest system, exposing the employee to a 10 foot fall hazard, as observed on or about June 5, 2019.    In accordance with 29 CFR 1903.19, abatement certification and documentation is required for this violation.    Duane Bainbridge dba Bainbridge Brothers Contracting was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1334583, citation number 1, item number 1 and was affirmed as a final order on September 21, 2018, with respect to a workplace located at 233 Morea Road Frackville, PA 17931.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:    a) Residential Building, 138 West Spring Street, Frackville, PA - Employees accessed a seven (7) foot, one (1) inch high roof by stepping on the top step of a six (6) foot portable step-ladder exposing the employees to a fall hazard, as observed on or about June 5, 2019.    In accordance with 29 CFR 1903.19, abatement certification is required for this violation.
