29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: (a) A work platform occupied by an employee was elevated by a rough terrain forklift and was not secured to the forks of the machine. This exposed the employee to a fall hazard of approximately 18.6 feet.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. (a) An employee conducting carpentry operations from a elevated platform supported by a Gradall 534C Rough Terrain Forklift was not protected from falling to the lower level. This exposed the employee to a fall hazard of approximately 18.6 feet.
