On or about June 5, 2019, an employee engaged in roofing operations was exposed to fall hazards in excess of 6-feet without using any form of fall protection.

On or about June 05, 2019, an employee engaged in roofing activities was exposed to fall hazards in excess of 6-feet while using a ladder that did not extend at least 3-feet above the roof's landing.
