White City Glass of Chippewa Falls, LLC, failed to provide documentation demonstrating the abatement method and completion date for the previously issued previously issued Citation 1, Item 1a, and Citation 1, Item 1b. The employer had not provided training to each employee utilizing self-supporting ladders and/or work benches on the proper usage, such as, but not including, not overextending and/or reaching out beyond the side rails of the ladder, not standing on the top cap and/or top step of any stepladder.
