GSW Masonry, LLC, 1525 West 8th Street, Erie, PA  16505, on or about June 5, 2019 and times prior thereto:  Employees were operating power equipment including, but not limited to, a Hilti TE 2 hammer drill with a flexible electrical extension cord.  The electrical equipment was not used in conjunction with a ground-fault circuit interrupter and/or an assured equipment ground conductor program.

GSW Masonry, LLC, 1525 West 8th Street, Erie, PA  16505, on or about June 5, 2019 and times prior thereto:  Employees were operating power equipment including, but not limited to, a Hilti TE 2 hammer drill with a flexible electrical extension cord.  The flexible electrical extension cord was running under a closed garage door and into the building to an electrical receptacle.

Cave-in protective systems were not utilized while work was being performed.
