29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:    (a)  Pallet Shop:  On June 5, 2019 and times prior, the employer has not developed a specific energy control procedure for the Keystone chop saw for employees that conduct servicing and maintenance, such as blade changes. The Keystone chop saw has multiple energy sources, 480 volt electrical and 120 psi pneumatic pressure.    (b)  Pallet Shop:  On June 5, 2019 and times prior, the employer has not developed a specific energy control procedure for the Keystone gang saw for employees that conduct servicing and maintenance, such as blade changes. The Keystone chop saw has multiple energy sources, 440 volt electrical and 120 psi pneumatic pressure.

29 CFR 1910.147(c)(6)(i):  The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed:  (a) Pallet Shop:   On June 5, 2019 and times prior, the employer did not conduct an annual periodic inspection between an inspector and authorized employee to ensure that the employee(s) followed the energy control procedures for the Keystone chop saw which has multiple energy sources; 480 volt electrical and 120 psi pneumatic pressure. (b) Pallet Shop:  On June 5, 2019 and times prior, the employer did not ensure that lockout devices were applied to the electrical disconnect (440 volt) and pneumatic pressure on the Keystone gang saw by the authorized employee during maintenance and servicing such as, changing the blade.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a)  Pallet Shop:  On June 5, 2009 and times prior, the employer did not ensure that machine guarding was provided on the Keystone chop saw to prevent employees contacting the bottom portion of the 20 inch circular blade during operation.
