29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided:    a) Warehouses at 16 Leliarts Ln and 307 54th St, Elmwood Park NJ : Operator of Power Industrial Truck (PIT) was operating equipment with defective seatbelt. On 6/6/19 worker was crushed to death while operating a PIT without the use of a seatbelt.
