29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):    (a)  Triple AAA Builders LLC, at 1100 Blue River Parkway, Silverthorne, CO: On and preceding 6/5/19, the employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them).  Employees were framing multi-story multi-family residential structures.  Employees were exposed to fall hazards and flying object hazards during work operations.  The employer did not conduct frequent and regular inspections of the jobsite.  This condition exposed the employees to approximate 22.5 foot and 10.1 foot fall hazards and eye injury hazards.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:    (a)  Triple AAA Builders LLC, at 1100 Blue River Parkway, Silverthorne, CO: On 6/5/19, the employer did not ensure that eye and face protective equipment were used when machines or operations presented potential eye or face injury.  Employees were framing a multi-story multi-family residential structure.  Employees used pneumatic nail guns to frame the wood structure.  Eye protection was not used during this operation.  This condition exposed the employees to an eye injury hazard.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure:    (a)  Triple AAA Builders LLC, at 1100 Blue River Parkway, Silverthorne, CO: On 6/5/19, the employer did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure.  Employees were framing a multi-story multi-family residential structure.  Approximately four employees were not protected from a fall while working on the top plates of the interior and exterior framing of the two story structure.  The employer did not ensure that all employees were protected by guardrail systems, safety net systems, or personal fall arrest systems.
