29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:   a.) Shingle Rite Installers, LLC, on or about June 7, 2019 at the address of 7340 Kings Park Rd. Toledo, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.  Shingle Rite Installers, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(1) in inspection number 1212640 citation 1 item 1a which became final order on May 10, 2017, in respect to the worksite at 2110 Maplewood Rd. Toledo, OH.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:  a)      Shingle Rite Installers, LLC on or about June 7, 2019 at the address of 7340 Kings Park Rd. Toledo, OH: The employer did not ensure employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while removing shingles on the back side of a residential structure. Employees were unable to identify fall hazards on the job site, exposing them to fall hazards of up to 11ft and 18ft respectively.   Shingle Rite Installers, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1) in inspection number 1212640 citation 1 item 1b which became final order on May 10, 2017, in respect to the worksite at 2110 Maplewood Rd. Toledo, OH.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Shingle Rite Installers, LLC, on or about June 7, 2019 at the address of 7340 Kings Park Rd. Toledo, OH: The employer did not ensure employees were protected from falls while performing roofing activities for a residential type structure.  Employees were exposed to up to approximately 11 and 18 falls to the ground below.  Shingle Rite Installers, LLC was previously cited for violations of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1212640 citation number 2, item number 1 and was affirmed as a final order on May 10, 2017 with respect to a workplace located at 2110 Maplewood Rd. Toledo, OH, and inspection number 549258, citation number 2, item 1, and was affirmed on December 3, 2012 with respect to a workplace located at 721 Colt Rd. Findlay, OH.
