The employer did not instruct employees in the recognition and avoidance of those trench excavation hazards while performing this work.

29 CFR 1926.95(a): "Application." On or about May 31, 2019, located at a residential jobsite, employees were exposed to struck-by hazards while working inside a trench excavation greater than 5 feet in depth without use of eye protection.
