29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Employees conducting roofing activities on 8:12 pitch residential roof were not protected from falling to the lower level by not using an adequate fall protection system. This exposed employees to a fall hazard of approximately 9 7/10 feet.  A Plus Roofing and Restoration LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1230220, citation number 1, item number 2/a and was affirmed as a final order on 11/13/17, with respect to a workplace located at 105 N State Street, Waupaca WI.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  The employer does not have a program to train employees to recognize fall hazards and take the appropriate actions to eliminate those hazards, and does not ensure the employees have been trained by a competent person to properly set up and use fall protection systems or to understand the other standards in this subpart.  This most recently occurred on June 7, 2019, at 3332 S. Pennsylvania Drive, Milwaukee, Wisconsin.  Employees conducting roofing activities on a 8:12 pitch roof were not trained to recognize and protect themselves from fall hazards.  Employees were exposed to a fall hazard of approximately 9 feet 8 inches.  A Plus Roofing and Restoration LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(2)(iii), which was contained in OSHA inspection number 1230220, citation number 1, item number 2/b and was affirmed as a final order on 11/13/17, with respect to a workplace located at 105 N State Street, Waupaca WI.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   (a) The portable extension ladder on the north side of the roof, and being used for access to the roof did not extend at least three (3) feet above the upper landing area.
