On or about June 07, 2019, and at times prior thereto, employees were exposed to a fall hazard while installing a new metal commercial roof system without fall protection.

On or about June 07, 2019, and at times prior thereto, employees did not have adequate Personnel Protective Equipment (PPE) for their eyes, while working on a commercial metal roof system.
