29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Employees working at or near equipment were exposed to struck-by, caught-in, and /or amputation hazards in that adequate guarding was not provided in the following instances:      a) Chilton Mark XX Die Cutter Press Machine located in the 7250 36x96 die cutter area - back side not guarded b) Blue lamination machine located in the 7300-Laminators area - inadequate guarding for point of operation   c) M & R Sportsman machine located in the 4580-6S-Auto area - back side not guarded  d) M & R Gauntlet Series II Machine located in the 4585-12S-Auto shirt printing area - back side not guarded  e) Wrap-O-Matic Automatic sheeting machine located in the 1000-Stock Sheeting area- inadequate guarding   f) Thieme machine located by ballast #2 in the 4750-Thieme machine area - inadequate guarding of in-running nip point

29 CFR 1910.334(a)(3)(i): A flexible cord used with grounding type equipment did not contain an equipment grounding conductor:  Employees working at or near the Guardian Power Products Heavy Duty 3 Speed Drill Press located in 7150 Crest 1 area were exposed to electric shock hazards in that the grounding pin was missing on the plug end of the cord.
