29 CFR 1910.134(c)(2)(i): The employer did not determine if the voluntary use of a respirator did not in itself create a hazard.  The employer is failing to provide employees with Appendix D for the voluntary use of N95 dust masks to fill out and have evaluated.  This was most recently documented on June 5, 2019 at Tourney Brothers located at 104 East 6th Street, Wood River, NE 68883.  Employees were not given to fill out Appendix D prior to their use of N95 disposable respirators being used on the shop floor.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch.  The employer is failing to maintain the bench grinder with 1/8" max gap on lower tool rest. This was most recently documented on June 5, 2019 at Tourney Brothers located at 104 East 6th Street, Wood River NE 68883.  The bench grinder was not correctly gapped on the lower tool rest creating a caught by hazard to the employees.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch.  The employer is failing to maintain the bench grinder so as the upper gap of machine guarding to wheel does not exceed 1/4". This was most recently documented on June 5, 2019 at Tourney Brothers 104 East 6th Street Wood River, NE 68883.  The bench grinder was not gapped correctly on the upper adjustable guard that protects debris and sparks from struck by hazards to the employees.
