This was most recently documented, on or about 6/12/2019, at the job site located at Rd N 80 (39.466304, -100.575538) Selden KS 67757. Employees were exposed to electrical shock hazards while utilizing a truck mounted Smeal Hydraulic 6T derrick. The employer did not provide adequate training to employees such as but not limited to calculating the working distance from energized overhead powerlines based upon the tools used, angles of approach, and the configuration of the jobsite when working near energized overhead power lines.

An employee operating a truck mounted Smeal 6T derrick raised the derrick structure within 10 feet of an energized overhead powerline.
