29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards while performing roofing operations.  This was most recently observed at 13618 Atwood Ave, Omaha, Nebraska on June 13, 2019 when the employer and his 3 employees were observed installing roofing materials to a residential structure with an eave height over 9' and they were not using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer is failing to protect employees from fall hazards while using ladders.  This was most recently observed at 13618 Atwood Ave, Omaha, Nebraska on June 13, 2019 when employees were observed using a ladder to access the roof of a residential structure and it did not extend 3' past the upper landing surface.
