29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about June 7, 2019 the employer failed to ensure that employees tasked with replacing roofing shingles on roof having a slope of 4/12 were protected from falling to the ground approximately 10-12 feet below. No conventional fall protection measures were taken including a safety monitoring system.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about June 7, 2019 the employer failed to ensure that employees were provided with or utilized personal protective eyewear while engaged in roofing activities. Employees were exposed using pneumatic nail guns without effective safety glasses.
