Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    On or about June 7, 2019 the employer failed to ensure that employees tasked with replacing roofing shingles on roof having a slope of 4/12 were protected from falling to the ground approximately 10-12 feet below. No conventional fall protection measures were taken including a safety monitoring system.
