29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:    (a) On or about June 11, 2019, an employee was exposed to a fall hazard in that he did not have a fall protection on while working approximately 15 feet up in an aerial lift.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a) On or about June 11, 2019, the employer did not ensure that employees working on top of a steel structure while installing plywood sheets were protected from an approximate 15 foot fall hazard.
