29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about June 13, 2019, at the worksite located at the Preserve Condominiums in Miami Shores, FL 33138, an employee without the means of a fall protection system was required to work on the edge of the porch roof, to perform touch-up painting operations was exposed to a fall hazard of 13 feet 8 inches, above the ground below.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:    On or about June 13, 2019, at the worksite located at the Preserve Condominiums in Miami Shores, FL 33138, the employer did not provide ladder training to an employee who was required to use an extension ladder while performing painting operations of a two story building was crossing a wide distance from a ladder to a porch roof exposing the employee to a fall hazard of 13 feet 8 inches.

29 CFR 1926.1053(b)(4): Ladders were not used for the purpose for which they were designed.    On or about June 13, 2019, at the worksite located at the Preserve Condominiums in Miami Shores, FL 33138, the employer did not ensure the ladder was properly positioned, an employee was exposed to fall hazard of approximately 13 feet 8 inches to a concrete surface below, while performing painting operations was using 24 foot extension ladder that was placed several feet away from the surface onto which the employee was to gain access to the area around the window, the employee stepped off the ladder then onto the roof above the entrance to the condominium.
