29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:    a) PHB, Inc., Machining Division / 8150 West Ridge Road, Fairview, PA: On or about June 13, 2019 and times prior thereto, the employer did not ensure that authorized employees conducting lockout/tagout for servicing and maintenance of machines that include, but are not limited to, CNC machines, parts washer and parts testing machines were trained in the use and location of procedures for the machines.

29 CFR 1910.147(c)(6)(ii): The periodic inspection certification of energy control procedures did not contain the required documentation on identification of the machine or equipment, inspection date, employees included, and the person performing the inspection:  a) PHB, Inc., Machining Division / 8150 West Ridge Road, Fairview, PA: On or about June 13, 2019 and times prior thereto, the employer did not ensure that employees permitted to perform servicing and maintenance on machines that include, but are not limited to, CNC, drilling machines, parts washer and parts testing machines, participated in certified periodic inspections of the energy control procedure. The employer did not maintain documentation of the inspections and employees did not remember a documented inspection.
