On or about May 15, 2019, and times prior thereto, the employer did not ensure that each forklift operator was trained and evaluated as specified in paragraph (l) exposing employees to struck-by hazards.

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by paragraph (l).    On or about May 15, 2019, and times prior thereto, the employer did not ensure that a contractor completed the training required by paragraph (l) prior to permitting the contractor to operate forklifts exposing employees to struck-by hazards.
