29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part.  The Roofing Zone on or about 11:00am, June 14, 2019 at 1610 Cooper Forest Park Road West, Lorain, OH:  four employee's were observed conducting commercial asphalt shingling activities. The employer did not ensure that a fall protection program was in place for employees that are exposed to fall hazards over six feet in commercial roofing.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  The Roofing Zone on or about 11:00am, June 14, 2019 at 1610 Cooper Forest Park Road West, Lorain, OH: The employer did not ensure that a fall protection training program was in place for employees that are performing asphalt shingling to a commercial roof. Employees were observed shingling a 5/12 pitched roof without the benefit of fall protection training from The Roofing Zone LLC.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  The Roofing Zone on or about 11:00am, June 14, 2019 at 1610 Cooper Forest Park Road West, Lorain, OH:  four employee's were observed conducting commercial asphalt shingling activities. The employer did not ensure employees were protected from falls while working above six feet from the level below. Employees were observed shingling a 5/12 pitched roof without the benefits of fall protection.
