In the sheet metal fabrication area, located on the east wall, where employees were exposed to caught-by and struck-by hazards while using an Amada Promecam press brake (Model HFB5020; SN HFB05020R970420) without a point of operation guard.    b. In the sheet metal fabrication area, located on the east wall, where employees were exposed to caught-by and struck-by hazards while using an Trumpf 1 TrumaBend V170 press brake (no serial number) without a point of operation guard.    c. In the sheet metal fabrication area, located on the east wall, where employees were exposed to caught-by and struck-by hazards while using a Bridgeport mill (SN 27070) without a point of operation guard.    d. In the sheet metal fabrication area, located on the east wall, where employees were exposed to caught-by and struck-by hazards while using a Bridgeport mill with a saw attachment (SN 136569) without a point of operation guard.
