The ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: The employer did not ensure portable ladders were constructed and placed so three (3) feet or more was extended above the upper landing. Workers performing commercial construction were exposed to falling approximately twelve (12) feet while using a "little giant" type ladder to ascend/descend from the roof to the ground.
