Each employee that was engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  The employer did not ensure appropriate fall protection was implemented for employees performing commercial roofing operations on a 4:12 pitch roof, with an eve height of approximately twelve (12) feet and a peak height of approximately twenty (20) feet.

29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper landing surface. The ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  The employer did not ensure portable ladders were constructed and placed so three (3) feet or more was extended above the upper landing.  Workers performing commercial construction were exposed to falling approximately twelve (12) feet while using a "little giant" type ladder to ascend/descend from the roof to the ground.
