This was most recently documented on or about June 18, 2019 at the Muse Apartments construction site located at 1005 N 20th St, Omaha, NE 68102. Two employees were not utilizing an approved anchorage point and were securing their lanyard to 2 x 4 wood framing by wrapping it around the framing and attaching the lanyard back onto itself.
