29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about 06/19/19, on a roofing jobsite, at a house at 19 Yale Place, Buffalo, NY: Three employees engaged in tear-off roofing work on the steep roof on the east side of the house were exposed to a fall of approx. 18 ft. because fall protection  was not being utilized. Pitch of the roof was approx. 14 in 12.  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about 06/19/19, on a roofing jobsite, at a house at 19 Yale Place, Buffalo, NY: An extension ladder was provided on the east side of the house for employees to access the roof. The side rails of that extension ladder were only approx. 16 inches above the roof.  NO ABATEMENT CERTIFICATION REQUIRED
