29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  MHR Construction LLC at 871 Colorado River Ave: On and before June 18, 2019, the exposing employer did not ensure that two employees were protected from injury, in that the employees, conducting framing work on the second story of a residential home, were not utilizing a fall protection system when working around fall hazards. This condition exposed the employees to a fall of approximately 10 feet 10 inches from the second story to the ground.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  (a)  MHR Construction LLC at 871 Colorado River Ave: On and before June 18, 2019, the exposing employer did not verify compliance for fall protection training by preparing a written training certification record for four employees exposed to heights greater than 6 feet, while conducted framing work at a residential home construction site. This condition may have contributed to two employees being exposed to a fall of approximately 10 feet 10 inches from the second story to the ground.
