29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:     On or about June 20, 2019, on the wooden roof trusses and roof of a structure under construction located at 11579 SW Discovery Way in Port Saint Lucie, Florida, employees were exposed to a fall hazard of approximately 11 feet when performing carpentry work installing plywood sheathing on roof trusses while not being protected from falling to the lower level.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about June 20, 2019, on site of a structure under construction located at 11579 SW Discovery Way in Port Saint Lucie, Florida, employees were exposed to a fall hazard when performing carpentry work installing plywood sheathing on roof trusses prior to being provided with a training program that would enable them to recognize the hazards associated with falls.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    On or about June 20, 2019, on the interior concrete tie-beam edge of a structure under construction located at 11579 SW Discovery Way in Port Saint Lucie, Florida, employees were exposed to a fall hazard of approximately 11 feet when gaining access to and from roof by using an extension ladder that did not extend at least 3 feet past the upper landing surface and was not secured to prevent accidental displacement.
