29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  The employer is failing to ensure that the eyes of employees are protected from flying particles while working on a residential roof. This most recently occurred on June 19, 2019 on the jobsite located at 3030 South 137th Street, Omaha, NE 68104.  Employees were observed engaged in removal of roof material on a residential home without any form of eye protection exposing them to eye injuries.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards when conducting roofing operations on residential structures. This was most recently documented at 3030 South 137th Street, Omaha Nebraska on or about June 19, 2019 when employees were observed conducting roofing operations on a residential structure while working greater than 18 feet above the ground without fall protection.
