29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    a) 35283 Atlantic Ave, Millville Boardwalk, Millville, DE: On or about June 21, 2019, the employer permitted employees to use a JCB 506C All-Terrain Industrial Forklift with a wooden platform that was not designed by the manufacturer for use to elevate personnel, exposing them to falls.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:   a) 35283 Atlantic Ave, Millville Boardwalk, Millville, DE: On or about June 21, 2019, the employer permitted employees to use a JCB 506C All-Terrain Industrial Forklift with a platform that was not attached to the fork and forklift for use to conduct framing activities, exposing them to falls.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a) 35283 Atlantic Ave, Millville Boardwalk, Millville, DE: On or about June 21, 2019, the employer failed to ensure employees used fall protection while conducting framing work over 10 feet above a lower level from a JCB 506C all-terrain industrial truck, exposing them to falls.
