29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about 6/19/2019, at Ocean's Edge Resort and Marina, 5950 Peninsular Ave, Key West, FL 33431. Front of Building #2, two employees were exposed to a fall hazard of up to 12 feet to ground level while using a ladder that extended 1-1/2 feet approximately above landing surface.
